Abstract A 38-year-old G5P2 with previous 2LSCS, 2D&Cs and hysteroscopic myomectomy presented with a live Cesarean scar ectopic of 7 weeks gestation. Transabdominal ultrasound-guided intrasac potassium chloride and methotrexate instillation was performed under local anesthesia leading to a successful resolution of the ectopic pregnancy. diameter was 29 mm and crown to rump length was 14.8 mm both corresponding to 7 weeks and 5 days. Fetal heart rate measured 148 beats per minute. Transabdominal ultrasound-guided intrasac potassium chloride (KCL) and methotrexate instillation was performed under local anesthesia in the same sitting (Fig. 1c,d). Preprocedure betahCG of 1,65,000mIU/mL decreased to \ 5mIU/mL over 5 weeks. Ultrasound findings took 16 weeks to resolve completely (Fig. 1e,f).
