Neutropenia in renal transplant recipients: 2 case reports Two renal transplant recipients developed neutropenia after treatment with valaciclovir and valganciclovir [route, therapeutic indication and duration of treatment to reaction onset not stated]. Patient 1 was an 18-year-old adolescent male who developed fatigue and loss of appetite 2 months after a renal transplant. His medications included mycophenolate mofetil, ciclosporin and valganciclovir 900 mg/day. Laboratory investigations revealed a leukocyte count of 1.7 × 109/L. Discontinuation of mycophenolate mofetil and valganciclovir failed to improve his neutrophil count, which did not rise above 0.1 × 109/L, so he was treated with a granulocyte colony-stimulating factor. In 24 hours, his leukocyte count improved to 6.5 × 109/L and his neutrophils increased to 2.2 × 109/L. Patient 2, was a 37-year-old man who developed fatigue and burning sensations in his stomach 3 months following a renal transplant. His medications inlcuded mycophenolic acid, tacrolimus, and valaciclovir 4.5 g/day. Laboratory investigations revealed a leukocyte count of 1.3 × 109/L. His leukoopenia persisted despite mycophenolic acid and valaciclovir discontinuation; his leukocyte and neutrophil counts failed to rise above 1.4 × 109/L and 0.1 × 109/L, respectively. He received a granulocyte colony-stimulating factor, and after his third dose, his neutrophils increased to 3.8 × 109/L and his leukocytes to 5.8 × 109/L. Author comment: "In our patients, neutropenia was related to the use of valacyclovir and valganciclovir; there was no evidence of an infection, and granulocyte colony stimulating factor was used to treat persistent neutropenia, despite discontinuation of antiviral agents."
