Vemurafenib Hidradenitis suppurativa, eruptive melanocytic nevi and keratosis pilaris-like eruption: case report A 51-year-old man developed hidradenitis suppurativa, eruptive melanocytic nevi and keratosis pilaris-like eruption while receiving vemurafenib for metastatic cancer. The man had a history of a nodular melanoma on his scalp which was treated with wide local excision and radical lymph node dissection. He received 1 year of high-dose interferonα-2b in 2009. Recurrent lymphatic disease was detected, and multiple chemotherapy regimens failed to halt disease progression. His tumour tested positive for the BRAF V600E mutation and he was initiated on vemurafenib [route and dosage not stated] in October 2011. Ten weeks later, follicularbased open and closed comedones and cysts appeared on his earlobes, cheeks and postauricular ears. After 3 weeks, he returned with numerous hyperkeratotic open comedones on his scalp. Painful draining nodules and multiheaded open comedones were also present in his axilla. After 1 month, increasing hyperkeratotic, follicular-centred papules developed on his chest, forearms, proximal thighs, buttocks and lower back. Numerous nevi emerged on previously unaffected skin. The man’s hidradenitis suppurativa was moderately improved by benzoyl peroxide and doxycycline, and his scalp lesions were improved with tretinoin cream. Vemurafenib was stopped due to disease progression and his cysts, keratosis pilaris-like eruption and hidradenitis suppurativa almost completely resolved, while the nevi persisted.
