Methods: We analysed 1200 clinical folders of patients died in Pio Albergo Trivulzio between 2004 and 2005. We selected 502 patients (163 Men and 339 Women) aged over 80 years (M 86±4.44; F 88±5.14) and considered causes of death, concomitant disease and major cardiovascular risk factors. Death causes was divided into 6 groups: cardiovascular causes, (MCV: cardiac arrest, ischaemic heart disease, arrhythmia, pulmonary oedema, heart failure), respiratory causes (MP: respiratory failure, pulmonary embolia), cerebrovascular causes (McerV: stroke, cerebrovascular disease), tumour (NPL), infective causes (INF), and various. We have also investigated the presence of hypertension, diabetes, renal failure, and dyslipidaemia. Results: In 39.29% of patients (M 32,52%;F 42,48%) the main determinant of death was MCV, followed by NPL (23.11%(M 10,43%; F 21.24%), MP (M 6.13%; F 10,62%), McerV in 8.57% (M 10.43; F 7.67%) and INF in 5,18% (M 6,13%; F 4,72%). About the prevalence of the main risk factors, 55,38% of patients suffer from hypertension (M 53,99%; F 56,05%), 41,24% of patients suffer from a tumour (M 49,08%;F 37,46%); diabetes mellitus was present in 18,33% of patients(M 22.08%; F 16.52%) and 30.08% suffer from chronic renal failure (M 39.26%; F 25.66%). All hypertensive patients takes antihypertensive drugs whit values of SBP 127.8±17.4 mmHg and DBP 73.8±8.6 mmHg. Only 23% of patients had a complete lipid profile, 27% had an incomplete profile and 50% had no values of cholesterol. Conclusions: Arterial hypertension and chronic renal failure are the most frequent risk factor in very old patients; tumour, cardiovascular and cerebrovascular disease are the main determinant of death.
