First report of an interaction, leading to hypotension: case report A 62-year-old White man developed hypotension during treatment with alfuzosin and vitamin K2 [menaquinone; Solgar]. Vitamin K2 100µg daily [route not stated] was added to his existing coronary artery disease medications including alfuzosin 10mg daily. About 2 hours after starting vitamin K2, he developed dizziness and weakness; his BP was 110/55mm Hg. Over the next 2 days, his BP was 105/50mm Hg and 100/50mm Hg, respectively, 2 hours postadministration. Vitamin K2 was withdrawn, and the man’s BP improved to 115/65mm Hg on day 4 before normalising. Ten days after vitamin K2 withdrawal, the man was rechallenged. Two hours postadministration, his BP was 115/60mm Hg, which fell to 100/55mm Hg the following day. Vitamin K2 was withdrawn again [outcome not stated]. According to the Naranjo adverse drug reaction probability scale, menaquinone was found to be the probable cause of his hypotension (score of 7). Author comment: "The drug interaction probability scale score was calculated and revealed that a drug interaction was the probable cause of the patient’s hypotension (score of 6) . . . Editorial comment: A search of AdisBase, Medline and Embase did not reveal any previous case reports of an interaction between alfuzosin and vitamin K2. The WHO ADR database contained no reports involving the concomitant use of alfuzosin and vitamin K2.
