Amitriptyline Hypomanic symptoms and excessive drowsiness: case report A 55-year-old woman developed hypomanic symptoms and excessive drowsiness during treatment with amitriptyline for central post-stroke pain and depression. She received oral amitriptyline 25mg once daily. After 4 months, she began demonstrating hypomanic symptoms, including an elated mood and increased appetite. She also reported drowsiness. Her amitriptyline was withdrawn. Her hypomanic symptoms and drowsiness persisted 2 weeks later. She giggled for no apparent reason and believed other people could read her mind. She was referred for psychiatric evaluation and diagnosed with adjustment disorder. This diagnosis could not be confirmed because follow-up was discontinued. Author comment: "Amitriptyline worked well for this patient, . . .However, over time, she developed excessive drowsiness and mood changes." Tan EC, et al.
