Calf Liposuction

The level of satisfaction of calf liposuction is lower than for other areas as the calf has less fat compared with the amount of muscle. In most cases, there is a small amount of fat in the calf. Many women, however, undergo calf liposuction as they want slimmer calves and ankles. The author does not recommend calf liposuction as the legs appear to be in a straight line and the calves also look slimmer if the patients undergo thigh liposuction. The indication for operation should be set by finding out the degree of development of the musculoskeletal structure of the calf , because the level of satisfaction can be low after liposuction if the musculoskeletal structure is developed, even though there is fat in the calf. I measure the fat thickness of the calf using ultrasonography (USG). I consider performing calf liposuction if the fat thickness is more than 1.2 cm in an area with the most fat when standing and the fat thickness if more than 1.5 cm in a lying position without the calf muscle tensed.

I do not see any cases where the gastrocnemius muscle at the back of the calf and the soleus muscle at the lateral calf are developed as showing much of an indication for liposuction. In the case of patients with developed calf muscles, I used to conduct muscle involution by blocking the nerve. Now, I reduce the developed gastrocnemius muscle by using Botox. The biggest cosmetic complication of calf liposuction is the scar left at the incision site as the incision site is in area that attracts attention. When performing calf liposuction, fat may be suctioned from the whole calf, except for the front of the calf with the ankle or from the ankle. In the case of ankle liposuction, you need to carry out suction from the lateral side of the Achilles tendon to an area below the medial gastrocnemius muscle, inducing a natural line.

Design

This is designed to conduct suction in the whole calf except for the anterior calf with the ankle (Fig. 7.2). When suctioning ankle only, perform suction in the area marked as the lateral side of the ankle.

Anesthesia

For anesthesia in calf liposuction, I use sleep anesthesia using propofol. At the initial stage of sleep anesthesia, I begin by using ketamine 0.4 mL + propofol 7–8 mL. Propofol is injected intermittently 4 min apart.
