Delirium: case report A 16-year-old boy developed delirium during treatment with cefalexin for nail fringe inflammation [duration of therapy to reaction onset not stated]. The boy, who had a history of delayed development and learning difficulties, started receiving a course of cefalexin 750mg twice daily. One week later, he was admitted to an emergency clinic. His parents reported that following cefalexin initiation, he had become weary and forgetful with a tendency to stare and look vacant. His immediate memory had also become impaired and he had not slept well for some time. For the 24 hours prior to admission, he was tired and confused. On examination, he reported that sounds disturbed him and that recently he wanted to be left alone to ruminate over his affairs. He also demonstrate mild motor restlessness and tearfulness. Psychosis was suspected and he was hospitalised. Cefalexin was discontinued and the boy quickly felt better and, within 2 days, his delirium and psychosis symptoms resolved. After a couple of days of monitoring, he was discharged. During 7-months’ follow-up, he remained in good condition. Penttil¨a J. Delirium in an adolescent patient with the use of cephalexin.
