Differential Serodiagnostics of Latent Stages of Syphilis Based on Measuring IgG and IgM Levels towards Extended Panel of Recombinant Antigens of T. pallidum A. V. Runina, M. V. Shpilevaya, G. L. Katunin, and A. A. Kubanov

Translated from Byulleten’ Eksperimental’noi Biologii i Meditsiny, Vol. 169, No. 4, pp. 452-456, April, 2020 Original article submitted October 15, 2019 Immunochips containing 12 recombinant antigens of T. pallidum (Тр15, Тр17, Тр47, TmpA, Тр0163, Тр0277, Тр0319, Тр0453, Тр0684, Тр0965, Тр0971, and Тр1038) were prepared to assay for IgG and IgM in serum samples (n=68) of healthy individuals and patients with the latent stages of syphilis. The linear discriminant analysis of detected IgG and IgM differentiated three groups of serum samples as 1) early latent syphilis; 2) seroresistant early latent syphilis; and 3) late latent syphilis with overall differentiation potency of 95.6% (88.9-100%). The samples of all syphilis patients were differentiated from the samples of healthy individuals with 100% specificity. Key Words: recombinant antigens; T. pallidum; differential serodiagnostics; immunoarray; syphilis Syphilis is a sexually transmitted disease of great social importance that is characterized by a long chronic course and severe consequences for patient’s health [2]. In recent years, the incidence of syphilis in Russian Federation has shown a decrease, but the rate of latent and late forms of the disease has increased and exceeded 60% of total cases, as reported by Russian Federal State Statistics Service [3-5]. The latent stages of syphilis are diagnosed in individuals, who had not received antisyphilitic therapy and demonstrate no clinical manifestations of this disease, but are positive according to the results of serological tests. According to ICD-10, early latent syphilis has no clinical manifestations and is characterized by positive serological reaction, negative spinal fluid test, and a period less than 2 years after infection. Epidemiologically, the patients with early latent syphilis should be considered as contagious because they can develop clinical manifestations of the disease and transmit the infection. syphilis in ICD-10 is similar, but its duration is more than 2 years. Epidemiologically, the late stage of latent syphilis is viewed as non-contagious. However, contagiosity of patients with late latent syphilis of 2-3-year duration is still under discussion [8].
