Diltiazem Acute generalised exanthematous pustulosis and maculopapular exanthema: 3 case reports

In a case series, 3 women in their 60s [exact ages not stated] were described, who developed, of whom, two women developed acute generalised exanthematous pustulosis (AGEP) and one woman developed maculopapular exanthema (MPE) during treatment with diltiazem [dosage, route, indication,times to reaction onsets and outcomes not stated]. Case 1: The woman developed AGEP following administration of diltiazem. Six weeks after an AGEP episode, a patch test performed on the unaffected upper back skin with diltiazem was positive (+++) on day 2. On day 4 of patch test, she developed an angry back reaction associated with MPE involving her face, neck and armpits, without systemic symptoms. Eventually, a diagnosis of AGEP secondary to diltiazem was confirmed. Case 2: The woman developed MPE following administration of diltiazem. Four months after a MPE episode, a patch test with diltiazem was positive (+++) on day 2, spreading widely on her back on day 4. Eventually, a diagnosis of MPE secondary to diltiazem was confirmed. Case 3: The woman developed AGEP following administration of diltiazem. Three months after an APEG episode, a patch test with diltiazem was positive (++) on day 2 and became pustular on day 4.
