Anaphylaxis: case report A 56-year-old woman developed anaphylaxis following treatment with dipyrone for pain associated with liver cirrhosis secondary to hepatitis-C virus (HCV) infection. The woman, who had HCV liver cirrhosis, developed a severe immediate allergic reaction after receiving IV dipyrone [metamizole; dosage not stated] for controlling the pain, at the end of endoscopy. Within a few minutes, she developed tachycardia, severe hypotension and severe bronchospasm, and she became unconscious. The findings were suggestive of metamizole anaphylaxis. The woman was subsequently transferred to the ICU for emergency treatment. She recovered completely within a few days. Previously, she had experienced various episodes of angioedema of eyes and lips following administration of various NSAIDs including paracetamol [acetaminophen]. She underwent endoscopic banding under propofol sedation. However, during the procedure, she had uncontrolled urinary loss and vomiting. Subsequently, she received treatment with unspecified antiemetic drugs and proton pump inhibitor. She was scheduled to received pain medication, for which, she underwent intradermal test for morphine (result was positive). Following a positive intradermal skin test with nefopam, she tolerated nefopam dose during drug provocation test. Bumbacea RS, et al.
