Epinephrine Ischaemia following accidental autoinjection treated with papaverine: case report A 20-year-old female laboratory technician accidentally punctured her right thumb while handling an epinephrine autoinjector [Epipen]; a total amount of 1:1000 epinephrine [adrenaline] 0.3mL was injected. Her finger became pale and cold, pain developed, and capillary refill time was prolonged to 5 seconds [time to reaction onset not stated]. Immersion in warm water was unsuccessful, and papaverine chloride 20 mg/mL was administered to her right thumb with a sponge for 3 hours. Her pain resolved, and her finger returned to its normal appearance; capillary refill time also normalised. She completely recovered, with no disability apart from local tenderness. Baris S, et al.
