Lack of efficacy: case report A 71-year old man experienced lack of efficacy following treatment with epinephrine and unspecified antihistamines for respiratory distress secondary to angioedema of tongue, face and lips. The man presented at an emergency department with angioedema (without urticaria) of tongue, face and lips following 4 years of unspecified angiotensin converting enzyme inhibitors therapy. He developed respiratory distress in the emergency department which was not responsive to epinephrine or unspecified antihistamines [dosages and routes not stated]. C1 esterase and complement (C3/C4) levels were found to be normal. The man received C1 inhibitor concentrate. Within 1h of administration, there was a significant improvement in oedema and resolution of respiratory distress. Lohrenz S, et al.
