1. Antineoplastics Cyclophosphamide/Melphalan: Early-Onset Acute Lymphoblastic Leukemia in a Patient with Multiple Myeloma: Case Report

A 61-year-old woman with multiple myeloma developed acute lymphoblastic leukemia (ALL) with mixed-lineage leukemia gene (MLL) rearrangement after receiving cyclophosphamide and melphalan [durations of therapies not stated]. The woman presented in January 2007. She was diagnosed with multiple myeloma; she had a normal karyotype (46,XX[20/20]). In July 2007, following 4 cycles of high-dose dexamethasone, her peripheral blood stem cells were mobilized and collected using cyclophosphamide 4 g/m2 and granulocyte colony-stimulating factor. In August 2007, she underwent autologous stem-cell transplantation with high-dose melphalan (200 mg/m2) as a conditioning regimen. She achieved partial remission. However, in July 2008, pancytopenia was detected. Her bone marrow had 94% blasts with the immunophenotype as B-cell lymphoblasts. Chromosomal abnormalities were evident: 46,XX,t(1;11)(p32–34;q23) [4/6]. Split signals of MLL gene were observed upon fluorescent in situ hybridization. She was diagnosed with B-ALL with MLL rearrangement. The woman was started on the JALSG
