Gadolinium-containing contrast agents Nephrogenic systemic fibrosis: 2 case reports Two haemodialysis-dependent patients developed nephrogenic systemic fibrosis (NSF), with periocular papules, after receiving gadolinium-containing contrast agents [specific compounds and dosages not stated] for MRI examinations. [Treatments and outcomes not stated.] A 42-year-old woman with focal segmental glomerulonephritis, received a gadolinium-containing contrast agent during a brain MRI scan. About 2 months later she not only developed hyperpigmented, sclerotic plaques on her lower and upper extremities, but also solitary, bilateral, skincoloured papules approximately 1cm from her lateral canthi. Lesion biopsy findings were consistent with NSF by showing prominent collagen bundles with mucin and a localised proliferation of spindle shaped cells in the subcutaneous tissue. A 60-year-old woman with adult polycystic kidney disease, underwent pelvic MRI scanning with a gadolinium-containing contrast agent. Approximately 6 months later, NSF was diagnosed after she presented with skin-coloured papules about 2cm from her lateral canthi. Histopathological results from a lesion on her arm were also indicative of NSF, as was fibrosis of her knees and ankles which caused sclerosis of her joint contractures and upper extremities.
