Torsade de pointes: case report A 52-year-old man developed torsade de pointes while receiving haloperidol for schizophrenia [duration of treatment not stated]. The man, who had a 5-year history of schizophrenia, was receiving haloperidol at a dosage of one 5 mg tablet three times daily. He presented to hospital after a syncopic episode, and reported dizziness and darkening vision. He had had another syncopic episode the previous day. An ECG was consistent with torsade de pointes, showing ventricular tachycardia with modified width of the QRS interval with a sinusoidal appearance, broadened ventricolograms and paroxysms, irregular R-R intervals and ventricular frequency of 200–250/min. His potassium level was 2.8 mEq/L. The man received IV magnesium sulphate and potassium chloride. Haloperidol was discontinued.
