Herbal medicines Acute generalised exanthematous pustulosis in an elderly patient: case report A 65-year-old man took two herbal medications of unknown composition and dosage for general well-being and, 3 days later, he developed a skin eruption. At presentation 2 days later, examination revealed extensive eruption of an oedematous erythema that was studded with small nonfollicular pustules measuring < 5mm. He had a fever (38.5°C). Laboratory analysis showed hypocalcaemia (2.0 mmol/L), hypoalbuminaemia (38 g/L), and leucocytosis (12.2 × 109/L) with neutrophilia (80%). Histopathological analysis of a skin biopsy sample was consistent with acute generalised exanthematous pustulosis. Within 2 weeks, his skin lesions resolved completely without any specific treatment. At follow-up 2 months later, he was healthy without lesion recurrence. Author comment: "The fact that this patient’s history was negative for any illness or drug intake during the last 4 weeks, except for the herbal treatment, leads us to strongly suspect herbal medicines to be the cause of [acute generalised exanthematous pustulosis] in this case."
