Iron Iron pill gastritis: case report A 23-year-old woman developed iron pill gastritis during treatment with iron supplementation for iron-deficiency anaemia. The woman, who had iron-deficiency anaemia, presented with abdominal pain. She had developed flu-like illness, for which she recently took an unspecified NSAID. She had been on iron supplementation in the form of oral iron pills [dosage not stated] from the previous month. When presented, she underwent GI endoscopy. A biopsy of the lesion from abnormal region demonstrated gastric antrum with an area of mucosal pallor. Histological examination revealed features of a chronic gastritis, reactive gastropathy and iron deposition within the surface epithelium, consistent with a diagnosis of iron pill gastritis [time to reaction onset not stated]. Subsequently, the woman’s iron pill treatment was discontinued which was switched to liquid/parenteral formulation. Afterwards, at a follow-up, she was asymptomatic with normal haemoglobin levels. Author comment: "Iron pill gastritis is a recognised complication of iron supplementation ... The diagnosis should be considered when unexpected abnormalities are found in the upper GI tract, in patients receiving oral iron therapy".
