Lamotrigine Exacerbation of psychotic symptoms in patients with schizophrenia: 3 case reports Three patients with paranoid schizophrenia experienced an exacerbation in their positive psychotic symptoms, while being treated with lamotrigine. A 23-year-old man had been treated with risperidone during a recent relapse involving hallucinations and delusions. This was then augmented with lamotrigine 150 mg/day. Six weeks later, he experienced an exacerbation of his positive symptoms (Positive and Negative Syndrome Scale [Positive subscale; PANSS-Pos] score of 28). Lamotrigine was discontinued immediately, and his positive symptoms improved over the next 2 weeks. A 40-year-old man had been experiencing mild positive symptoms under haloperidol treatment, when he was initiated on lamotrigine 150 mg/day as a mood stabiliser and antidepressant. Six weeks later, he experienced a relapse of his positive symptoms, with severe delusions and auditory hallucinations (PANSS-Pos score of 25). Lamotrigine was discontinued immediately, and his delusions and hallucinations improved over the next 2 weeks. A 38-year-old man with auditory and visual hallucinations, was supplemented with lamotrigine 100 mg/day, in order to improve an inadequate response to his olanzapine treatment. Four weeks after initiating lamotrigine, his positive symptoms worsened (PANSS-Pos score of 24). Lamotrigine was discontinued immediately, and the hallucinations improved over the next 2 weeks. Author comment: "According to Naranjo’s criteria for causal attribution of adverse drug reactions, [lamotrigine] constituted a possible cause of patients’ psychotic exacerbation."
