Large-Scale Clinical Data Management and Analysis System Based on Cloud Computing Ye Wang, Lin Wang, Hong Liu and Changhai Lei
Abstract With the exponential increase of Electronic Health Record systems in China, large-scale clinical data management and analysis have become big challenges. This paper depicts a novel system named Clinical Data Managing and Analyzing System, which uses hybrid XML database, and HBase/Hadoop infrastructure to handle big amount of heart disease clinical data analysis online. Using standardized format of Clinical Document Architecture, the system now has integrated more than 50,000 valvular heart disease clinical documents and provided efficient distributed data mining tools as well as data managing tools for doctor users from multi heart clinical centers in six different 3A hospitals of China. 177.1 Introduction Recently, Electronic Health Record (EHR) in China is increasingly regarded as the most important data basic for enhancing the quality of nation-wide health care. More and more paper based medical records are now being converted and archived as EHR documents and being shared across the nation on the Internet [1]. As a major part of EHR documents, clinical data and the related medical follow-up data are increasing explosively as well. In China, these electrical documents are often inputted, stored and managed by appointed nurses in the hospitals. However, there are too many different kinds of standards, protocols and applications in different Y. Wang H. Liu C. hospitals and different fields. It is still hard to manage and share the clinical data on Internet among doctors or nurse users from all over China. From 2010, working for the project ‘‘Standardized Surgical Treatment of Valvular Heart Disease’’ funded by Chinese Ministry of Health (MOH), our research group helped six 3A hospitals’ departments of Cardiothoracic Surgery in three major cities of China collaborate to establish a heart disease clinical documents managing, sharing and analyzing platform. These six hospitals are Changhai Hospital and Zhongshan Hospital in Shanghai, Fuwai Hospital, Anzhen Hospital and PLA 301 Hospital in Beijing, and People’s Hospital in Guangzhou. Our working group now has gathered more than 50,000 valvular heart disease clinical documents and the related follow-up data forms for more than 20 years (from 1991–2012), and these data are still increasing rapidly every day. When we were designing and implementing this Clinical Data Managing and Analyzing System (CDMAS), we focused on three major concerns as below. 1.
