Levosimendan Pulmonary hypertension in an elderly patient: case report A 70-year-old man, who had severe aortic valve stenosis and coronary artery disease, underwent aortic valve replacement and a double coronary bypass. During his second hour in ICU, because of pre-existing heart failure, he started receiving levosimendan at a dose of 24 µg over 10 minutes followed by 0.1 µg/kg/min in a continuous infusion over 24 hours. After the initial loading dose and about 2 hours of treatment, his pulmonary artery pressure increased markedly. Within half an hour, his mean pulmonary artery pressure had increased to 53 mmHg from a baseline value of 29 mmHg. Levosimendan was stopped and, within 1 hour, his pulmonary artery pressure normalised without specific treatment. His postoperative course was uneventful.
