Various toxicities following drug intoxication: case report A 35-year-old man developed pulmonary and brain oedema, anoxia, right eye contusion secondary to drug intoxication following an overdose and abuse of methadone. Eventually, he died [route, dosage and times to reactions onsets not stated]. The man, was found dead at the rental home with an unknown cause. The postmortem (autopsy) revealed a mild contusion at the right eye. Autopsy confirmed that death was not related to any injuries or diseases. Microscopic pathologies revealed pulmonary and brain oedema caused by anoxia. Blood, vitreous humors, urine, and gastric content were also analysed. The toxicology tests (liquid chromatography-mass spectrophotometry-mass spectrophotometry) revealed blood methadone concentration was at the lethal levels in the blood and urine samples (drug intoxication). The other addictive substances, such as heroin, morphine, cocaine or ethanol, were not detected during investigation. The blood concentrations of methadone and its metabolite 2-ethylidene-1, 5-dimethyl-3,3-diphenylpyrrolidine (EDDP) were 1.9 mg/L and 0.058 mg/L, respectively. The concentration ratio of methadone and EDDP was 1.9:0.058=32.8 which was too high. Based on these findings and clinical presentation, the cause of death was confirmed as pulmonary and brain oedema secondary to methadone toxicity. Narongchai P, et al.
