Methyl aminolevulinate Pain: case report A 56-year-old man experienced pain while receiving methyl aminolevulinate [Metvix] to treat Buschke-Lowenstein tumour. The man was admitted to a dermatology inpatient ward with a rapidly growing Buschke-Lowenstein tumour of the anal region. He underwent topical photodynamic therapy, with application of 16% methyl aminolevulinate [dosage not stated] to the lesions, and 5–10mm of surrounding skin. After 3 hours, the lesion was irradiated. He reported a tingling pain during the first photodynamic therapy session [time to reaction onset not stated]. The man was given oral gabapentin during his second and third photodynamic sessions, and his pain was successfully controlled. Author comment: "Patients frequently report that photodynamic therapy illumination is accompanied by a burning sensation and stinging pain; these appear to be the most common complications. The mechanism of this pain if not fully understood, but nerve stimulation and tissue injury might be involved". Chu G-Y, et al.
