Norgestimate and levonorgestrel: similar VTE risk The risk of nonfatal venous thromboembolism (VTE) appears to be similar with ethinylestradiol/norgestimate and ethinylestradiol/levonorgestrel use, and significantly increased with ethinylestradiol/desogestrel compared with ethinylestradiol/levonorgestrel use, according to US-based researchers. To compare the risk of nonfatal VTE in women receiving ethinylestradiol/norgestimate or ethinylestradiol/desogestrel with those receiving ethinylestradiol/levonorgestrel, for oral contraception, the researchers conducted a nested case-control study* using claims data from the PharMetrics database from 2000 to 2005. They identified women aged 15–39 years with (n = 281) or without** (1055) deep vein thrombosis or pulmonary embolism who were current users of one of the study drugs at diagnosis (index date); each combination contained ethinylestradiol 30µg except ethinylestradiol/norgestimate, which contained ethinylestradiol 35µg. Compared with current ethinylestradiol/ levonorgestrel users, current ethinylestradiol/ norgestimate users had a similar risk of nonfatal VTE (adjusted odds ratios 1.1; 95% CI 0.8, 1.5) and current ethinylestradiol/desogestrel users had a significantly higher risk (1.7; 1.2, 2.4). The incidence rates of VTE in women receiving ethinylestradiol/norgestimate, ethinylestradiol/desogestrel or ethinylestradiol/ levonorgestrel were 30.6 (25.5, 36.5), 53.5 (42.9, 66.0) and 27.1 (21.1, 34.3) per 100 000 woman-years, respectively. The incidence rate ratios for use of ethinylestradiol/norgestimate and ethinylestradiol/ desogestrel were 1.1 (0.8, 1.5) and 2.0 (1.4, 2.7), respectively, compared with the use of ethinylestradiol/ levonorgestrel. * funded by Johnson and Johnson ** matched by birth year and index date Jick SS, et al.
