For years my dad said “you should write a book.” He never really said what about, and I don’t think specific content mattered so much. It was more along the lines of what I could teach to others. When he learned of the plans for this publication he was so excited, in his own earnest, pragmatic, and understated way. He would have been so proud to have seen this come to fruition but unfortunately died during the writing process. Despite my best efforts, I watched his dignity, mental and physical strength get stripped during a prolonged hospital stay. He remained stoic throughout, a testament to his steadfast nature. My love of attention to detail and analytical thinking are gifts from him that I hope elevate the level of information presented in this book and enhance the knowledge of those that care for the sickest patients.
