Rabbit syndrome: case report A 56-year-old man developed rabbit syndrome (RS) after withdrawal of paliperidone. The man, who had schizoaffective disorder, was in a stable condition on paliperidone 3 mg/d [route not stated] and lithium. He had experienced no remarkable psychotic symptoms for > 1 year and so paliperidone was discontinued gradually. Two weeks later, he developed involuntary movements of his perioral muscles. The movements were rapid, fine, and rhythmic, occurred along a vertical axis, and had a frequency of approximately 5Hz. On the Abnormal Involuntary Movement Scale, he scored 4 of 4 on the ‘lips and perioral area’. Over a subsequent 2-week observation period, there was no improvement and no further deterioration. He was diagnosed with RS. The man received biperiden and there was complete and prompt improvement in his RS over the following days. In the subsequent months, his biperiden dose was reduced and his RS recurred. Continuous treatment with biperiden was required.
