Lack of efficacy: case report A 27-year-old woman exhibited lack of efficacy during treatment with aciclovir, ibuprofen, prednisolone and unspecified corticosteroids for Ramsay Hunt syndrome [routes not stated; not all dosages stated]. The woman presented with symptoms of otalgia and oedematous eczema in the left external ear in July 2018. Initially, perichondritis was presumed. However, she also had left hemifacial paresis and dizziness. Hence, she was admitted to another hospital. A detailed investigations confirmed the diagnosis of Ramsay Hunt syndrome. She was treated with prednisolone 30mg, ibuprofen 1800mg and acyclovir; however, no improvement was noted. Additionally, she received flucloxacillin, moxifloxacin and humidifying eye drops and left eye protection. She was discharged on reduced doses of oral unspecified corticosteroids. Three weeks later, an electromyogram showed a severe left peripheral facial paralysis without signs of any recovery. Hence, treatment with acupuncture and related techniques was planned. With a combination of general acupuncture, local electroacupuncture treatments, phytotherapy and stimulation with a plum blossom hammer, a progressive improvement was noted. She was discharged with mobility practically recovered on the left side of the face and correct palpebral closure. Giralt Sampedro I, et al.
