Lack of efficacy: case report A 66-year-old woman exhibited lack of efficacy during treatment with aspirin, clopidogrel, atorvastatin and dopamine for ST elevation myocardial infarction (STEMI) or cardiogenic shock*. The woman, who had hypertension, presented to hospital with sudden anterior chest pain. ECG demonstrated anterior STEMI. She exhibited hypotension, and she was intermittently in heart block. In addition, she developed cardiogenic shock. Therefore, she started receiving aspirin, clopidogrel and atorvastatin along with dopamine drip [routes and dosages not stated]. She subsequently underwent cardiac catheterisation. An intra-aortic balloon pump (IABP) was placed, but aortography showed the presence of an acute dissection. IABP was discontinued immediately. She then underwent repair of the ascending aorta and the aortic valve. However, in spite of surgery and treatment, the cardiogenic shock persisted. She subsequently developed multi-organ failure, and she eventually died [immediate cause of death not stated]. * Country of occurrence, reporter country and primary source country not stated.
