Tic disorder: case report A boy in his early first decade of the age [exact age at reaction onset not stated] developed tic disorder during treatment with aripiprazole and risperidone for autism spectrum disorder [routes not stated; not all duration of treatment to reaction onset stated]. The boy was admitted at the age of 2 years for indifference to external environment and difficulty in eye contact. He was diagnosed with autism spectrum disorder. He was started on risperidone 0.5mg twice daily. Subsequently, he developed mild tic disorder. At the age of 4 years, his treatment was changed to risperidone 0.5mg daily and aripiprazole 1mg daily as expected control on the symptoms was not achieved. Within two days of aripiprazole initiation, he developed hyperactivity, agitation and tics. Additionally, his autism spectrum disorder worsened. Therefore, he was admitted. Examination showed involuntary motor tics involving shoulder, head and face movements for 3-5 seconds without apparent purpose and periodicity. Differential diagnoses of Gilles de la Tourette syndrome and attention deficit hyperactivity were ruled out. Hence, a final diagnosis of severe tic disorder secondary to aripiprazole and risperidone was made. Total Yale Global Tic Severity Scale score was found to be 64. Therefore, aripiprazole was discontinued. Consequently, his hyperactivity symptoms, agitation and tics improved 3 days after treatment discontinuation. Risperidone treatment was continued to treat autism spectrum disorder. This resulted in continuation of mild tic disorder. Copur M, et al.
