Elevation of SGOT and SGPT: case report A report received from The Egyptian Pharmaceutical Vigilance Center described an 86-year-old man, who exhibited elevation of SGOT and SGPT following concomitant administration of atorvastatin and clarithromycin. This case was reported to the regional center in Alexandria. The man was admitted to the ICU due to pneumonia. On day 1 of the ICU admission, his liver enzymes were found to be normal. On 30 January 2019, he started receiving atorvastatin [Ator] tablet 40 mg/day [route not stated] for underlying hyperlipidaemia and clarithromycin [Klacid] IV infusion 500mg over one hour as an antibiotic therapy. On the next day after the administration of atorvastatin and clarithromycin, elevation of his SGOT and SGPT levels was noted. Therefore, the man’s clarithromycin treatment was switched to levofloxacin. Atorvastatin was temporarily stopped. It was considered that, the elevation of liver enzymes was due to interaction between atorvastatin and clarithromycin. Eventually, a slight decrease in his SGOT and SGPT levels was noted. Author comment: "The Reporter stated that Klacid (Clarithromycin) interacted with Statin (atorvastatin) and affected its metabolism thus caused an increase in the statin level in the blood which increases it’s side effects." "[O]n the Next day of administration the two drugs [Atorvastatin and clarithromycin], the patient’s Liver enzymes were elevated (SGOT = 81 and SGPT =104)". Egyptian Pharmaceutical Vigilance Center.
