Oesophageal, gastric and duodenal ulcers with bleeding: case report A 77-year-old woman developed oesophageal, gastric and duodenal ulcers with bleeding, during treatment with calcium polystyrene sulfonate for hyperkalaemia. The woman had a medical history of chronic kidney disease and hypertension. She presented to hospital with pneumonia. She was treated with calcium polystyrene sulfonate [Kalimate; Kayexalate] 30 g/d for hyperkalaemia [route not stated]. After two weeks, she developed melena. Her haemoglobin level was found to be 7.8 g/dL. Esophagogastroduodenoscopy exhibited multiple oesophageal ulcers, gastric ulcers and duodenal ulcers with blood clots. Gastric ulcer biopsy showed necrotic mucosa, fibrinoid necrosis of blood vessels and some basophilic angulated crystals embedded in the lamina propria and mucosal surface. After examinations, it was concluded that her oesophageal, gastric and duodenal ulcers were calcium polystyrene sulfonate-induced [outcome not stated]. Wu Y-H, et al.
