Capecitabine Takotsubo cardiomyopathy: case report
An 80-year-old woman developed Takotsubo cardiomyopathy during treatment with capecitabine for anal cancer. The woman was hospitalised with the complaint of chest pain on 15 August 2017. She had a history of anxiety and receiving unspecified benzodiazepines, she also had hypertension and hyperlipidaemia. Her chest pain started 4 hours before hospitalisation, severity was 8/10. She also experienced tightness in her chest associated with shortness of breath. Two month previously, she was diagnosed with anal cancer and started receiving oral capecitabine 4 days before hospitalisation (11 August 2017) [dosage not stated]. She had been receiving other concomitant medications. Her vital sign meaurement showed following: pulse rate 58 beats/min, blood pressure 132/78 mmHg and respiratory rate 24 breaths/min. ECG showed hyperacute T-waves in the anterior precordial leads. The woman was prescribed aspirin and given a heparin drip. Her cardiac catheterisation revealed non-obstructive coronary disease, she also had arterial stenosis. Based on further investigations, she was diagnosed with Takotsubo cardiomyopathy. She was treated with aspirin along with unspecified statin and β-blockers. On day 3, she was discharged and remained free from chest pain. She was advised to discontinue capecitabine and undergo follow-up echocardiography. On 28 March 2018, a telephone follow-up was performed and she stated that she was doing good and not receiving any oral chemotherapy. Subsequent echocardiogram showed normal ejection fraction with no wall motion abnormality. She continued aspirin therapy. Bhardwaj PV, et al.
