Dear readers, Welcome to this issue of European Surgery. Nowadays everybody talks about cluster formation. Unfortunately, nobody talks about the formation of clusters of intelligence, clusters of open minded conduct of reasoning (Figs. 1 and 2). Therefore, this issue of European Surgery orchestrates a colourful spectrum of papers related to various types of general surgery. May the papers foster the formation of clusters of intelligence and open minded reasoning (Figs. 1 and 2). One should critically review clusters of group think. Today the general surgeon faces a complex reality (Fig. 1). Time is short. Importance justifies the length of this Editorial. Actuality justifies the length of this Editorial. Watch out.
