Dear Editor, We have read with interest the paper by Noriega et al. (2020) entitled “Balneotherapy with a psychoeducation program for the promotion of balanced care in family caregivers of older adults” and we would like to make two comments: first, the medical spa is proven to be an ideal place to promote health education and second, to consider a treatment such as Balneotherapy, a series of requirements must be met: the use of medicinal-mineral water in a specific medical spa, the use of specialized techniques, and of course, these must take place over an appropriate number of sessions and days (Gutenbrunner et al. 2010). We also believe that the medical spa is a suitable place to implement health education (Desfour 2011; Sandrin and Roques 2019; Fernández-Torán 2020). This has been demonstrated, among others, by Hanh et al. (2012) and SchnebelenBerthier et al. (2019) in obese patients; Kwiatkowski et al. (2013, 2017) in breast cancer survivors; Blain et al. (2016) in musculoskeletal disorders; De Maricourt et al. (2016) in benzodiazepine withdrawal; Maitre et al. (2017) for the maintenance of physical activity after thermal treatment; and Guerrero and Calmette (2020) in chronic dermatoses. In Spain, the “Cuidarelax” program was set up that offers intensive psycho-social-educational interventions on weekends

with groups of a maximum of 30 people dedicated to caring for the elderly, in medical spas that allow disconnection and relaxation (Ortiz and Martinez 2019), which is the same as the one referred to by Noriega et al. (2020) in their work. However we wish to highlight the importance of the duration of the treatment in Balneotherapy and to do so, we have prepared Table 1 from the data provided by Noriega et al. (2020) on Balneotherapy in their work, the original papers presented by Fioravanti (2020) on the occasion of the recent Balneology special issue in this Journal, and also others from this year. An examination of the Table clearly shows that the duration of Balneotherapy treatment requires a minimum of 10 sessions, occurring over at least ten days, while the norm is two or three weeks with 10 to 21 sessions, as other papers have confirmed (Hernandez-Torres et al. 2004; Françon et al. 2008; Özkuk and Uysal 2019). According to what has been said, the data provided by Noriega et al. (2020) on balneotherapy are notoriously insufficient: they only refer to the medical
