Leflunomide Recurrent transient ischaemic attack: case report

An 82-year-old man developed recurrent transient ischaemic attacks (TIAs) during treatment with leflunomide for rheumatoid arthritis (RA). The man, who had controlled RA, presented with acute motor aphasia lasting 4h, which had resolved and recurred with a frequency of 2–3 events/week for one month. One month prior to the current admission, his treatment for RA was transitioned to leflunomide [dosage and route not stated] with cessation of prednisolone. At admission, he was diagnosed with recurrent TIAs [duration of treatment to reaction onset not stated]. Aphasia was suspected to have an ictal manifestation, and he underwent electroencephalographic monitoring, during which the symptoms recurred without correlation electrographic correlation. The TIA was suspected to be an adverse reaction of leflunomide. The man’s treatment with leflunomide was discontinued. The causal relationship between TIA and leflunomide was confirmed with Bradford Hill’s criteria, and Naranjo Adverse Drug Reaction Probability Scale with a score of 8. Thereafter, he had no recurrence of aphasia. His RA was controlled with methotrexate. Navarro CE, et al.
