Peginterferon/ribavirin Thrombocytopenia: 2 case reports Two patients developed thrombocytopenia while receiving peginterferon and ribavirin for chronic hepatitis C. A 35-year-old man began receiving peginterferon and ribavirin [dosages not stated] and, after 9 months, developed severe thrombocytopenia. His platelet count decreased to 1 x 109/L. His bone marrow had a slightly decreased amount of megakaryocytes. Peginterferon and ribavirin were discontinued. He started to receive IV immunoglobulins. His platelet count increased to 51 x 109/L but subsequently decreased to 8 x 109/L. Later, he received prednisone, and achieved a complete and sustained response 3 months after discontinuation of peginterferon and ribavirin. A 35-year-old woman started receiving peginterferon and ribavirin [dosages not stated] and, after 10 months, developed severe thrombocytopenia. Her platelet count decreased to 4 x 109/L, and she had metrorrhagia. She twice received platelet transfusions, with no significant recovery. Her bone marrow had a decreased amount of megakaryocytes. She was positive for antiplatelet antibodies. Peginterferon and ribavirin were discontinued. She received IV immunoglobulins and her platelet count increased to 23 x 109/L. However, it subsequently decreased to 6 x 109/L. She later received prednisone, and achieved a complete and sustained response 12 months after discontinuation of peginterferon and ribavirin.
