Remifentanil more likely to induce cough than alfentanil Although equipotent boluses of alfentanil and remifentanil both induce coughing, the incidence of cough is greater after administration of remifentanil, report investigators from South Korea. A total of 465 patients undergoing general anaesthesia for elective surgery were randomly assigned to receive either IV alfentanil 10 µg/kg (n = 152), IV remifentanil 1 µg/kg (150) or an equal volume of 0.9% saline, over 10 secs. Any episode of coughing over a 1-min period from the start of infusion was recorded, and classified as mild (1–2 coughs), moderate (3–5) or severe (> 5). The overall incidence of cough was higher in the two opioid groups than the saline group. Moreover, the incidence of cough was significantly higher in the remifentanil group than in the alfentanil group (26% vs 7.2%). There was no statistically significant difference in the severity of cough between the alfentanil and remifentanil groups, the investigators note. Cho HB, et al.
