This book is based on my PhD thesis. Writing a PhD is often portrayed as one of the most annoying activities (see www.phdcomics.com); however, this has not been true in my case. Perhaps I did something wrong, but I have to admit, I enjoyed (nearly) every minute of it. Now I would like to thank the people who made this possible.
