Abstract Background To investigate the influence factors of infection complications of transrectal ultrasound-guided transperineal prostate biopsy. Methods A total of 2192 patients who underwent prostate biopsy under transperineal prostate biopsy were analyzed retrospectively from December 2010 to May 2020.We collected the clinical characteristics and the incidence of complications, and used univariate and multivariate logistic regression analyses to analyze independent risk factors for infection complications after transperineal prostate biopsy. Results Univariate analysis showed that the following factors were associated with the infection complications: diabetes, bacterial prostatitis, history of urinary retention, history of urinary infection, and number of cores. Furthermore, multivariate logistic analysis revealed that diabetes (OR 2.037, 95% CI 1.143–3.572, P = 0.021) and history of urinary retention (OR 2.563, 95% CI 1.284–3.901, P = 0.013) were independent risk factors for infection complications after transperineal prostate biopsy. Conclusions Patients with diabetes and history of urinary retention were more likely to have infection complications after transperineal prostate biopsy. Introduction Prostate cancer is a global disease that seriously endangers men’s health. According to the latest data from the American Institute of Cancer Research, 1,762,450 new tumor cases have been diagnosed in 2019, of which 174,650 were prostate cancer, accounting for 9.91%, ranking the first in male malignancies [1]. In China, the incidence rate has risen to sixth [2]. Transrectal ultrasound-guided prostate biopsy (TRUSPB) has become the primary means of early diagnosis of prostate cancer [3]. There are two main pathways to biopsy: transrectal and transperineal. However, the complications of the transrectal biopsy are diverse, and include hematuria, infection, urinary retention, lower urinary tract symptoms, rectal bleeding, etc. [4]. one of the most serious complications, and the number of fatal infections increases over time [5–7]. Lindert et al. [8] reported that bacteriuria was detected in 44% of patients and bacteremia was detected in 16% of patients after transrectal biopsy. And several studies have showed that the incidence of infection complications was ranging from 0.5 to 9.3% [9–12]. Loeb et al. [13] reported that 4.2% of postoperative patients had fever and 0.8% required hospital admission. Wu
