Dr. Shaheer Ahmed Consultant – Emergency Medicine, MVRCCRI, Kozhikode ABSTRACT BACKGROUND/OBJECTIVE: CRS and HIPEC is a novel promising treatment for patients with peritoneal carcinomatosis. Published Indian data on postoperative ICU experience of these patients is limited. This observational study, over a period of 6 months was intended To observe the postoperative course in ICU of these patients. To observe the laboratory values, complications, management and outcome during the ICU stay. METHODS: All patients shifted in the immediate postoperative period to the Surgical Intensive Care Unit (SICU) after undergoing CRS plus HIPEC surgery in our institution were included in our study after obtaining written informed consent. The primary diagnosis, postoperative parameters, laboratory values, requirement of blood products, oxygen therapy, fluid balance, complications, mechanical ventilation (MV) and high-flow nasal oxygen therapy (HFNO) in SICU were noted. The statistical analysis of the data was performed using SPSS 21.0 (IBM). Results were expressed as mean ± standard deviation. OBSERVATIONS And RESULTS: Among the 30 female patients who underwent CRS plus HIPEC and shifted to SICU, Ca ovary was the diagnosis in 75% of the cases. The mean blood loss was 970±444ml. Postoperative day (POD) 0: 57% of cases were on MV with a mean FiO2 of 0.4. All cases had low ionized calcium levels (0.05).
Peritoneal Surface Malignancy Treatment in a Low Resource Setting – An Initial experience Background: Management of peritoneal surface malignancies need a different approach to achieve better results. CRS and HIPEC is a potentially curative treatment for selected patients with peritoneal metastases. In India it is a costly affair for most patients. Additionally there is prolonged learning curve for a center and surgeon too. Not only the surgeon but other specialities too need to be well versed in managing such patients. Namakkal is relatively less known third tier city in the southern state of Tamilnadu, India. Two third of the catchment population belong to low socio-economic status. We started cancer treatment programme in our center since December 2014. Data review of the initial years showed an unmet need for treating patients with peritoneal surface malignancies (PSM).
