Abstract. The author describes a revolutionary body contouring technique based on the surgical use of ultrasonic energy. It allows the selective destruction of only excess adipose tissue. The technique is based on three fundamental steps: (1) preparation of the areas to be treated with a large infiltration of a special solution, (2) treatment of the areas with ultrasonic energy through special titanium probes, (3) manual remodeling of the treated areas to eliminate the fluid from the bursted adipocytes (fatty acids). The advantages of this new technique are selective destruction of just the undesired tissues, elimination of the fluid from the adipose tissues, and the possibility of a real "lifting" of the skin of the treated areas, and a reduction of physical strain on the surgeon. The author has already treated more than 280 patients with excellent results and without contraindications or undesired side effects. The fundamental steps of ultrasonic liposculpturing, including a description of the physical principles on which the technique is based, are presented. It is almost 15 years since the surgical technique called liposuction was introduced into the daily practice of the aesthetic surgeons. After initial skepticism and perplexity, liposuction, which allowed the surgeon to fix "in a covered field" the unaesthetic appearance of lipodystrophy and localized adiposity, has met with universal approval and is the most performed aesthetic operation in the world. Over the years many authors have helped to standardize
and refine the technique, and many plastic surgeons have tried to modify its application and the equipment used in order to obtain better results [12, 14, 18, 20, 21, 39, 45]. Nevertheless, when Fisher [9] in 1976, expanding upon and improving the lipexeresis concept described by Schrudde in 1972 [37], got the idea of using a tube connected to a suction device to remove "in a covered field" -without any incision-the adipose tissue from some parts of the body, all subsequent changes and improvements have only marginally altered the original principle of this technique. In this article the author builds on the concepts outlined by Scuderi in 1987 [7, 38] and describes a totally new original technique for remodeling the body, based on the use of a new device that was developed for this purpose. This new technique, which is based on the surgical use of the ultrasonic energy, allows for the selective destruction of excess adipose tissue, or rather of the fluid fraction that represents about 90% of the adipose tissue's volume.
