First report of rhinosinusitis: case report A 55-year-old woman developed chronic rhinosinusitis during treatment with zolpidem for insomnia. The woman received long-term treatment with zolpidem [Ambien; dosage not stated]. She initially reported symptoms of sinus pressure, nasal congestion and headache [time to reaction onset not stated]. She had recurrent sinus infections with increased facial pain, postnasal drip, mucopurulent discharge, cough and low-grade fevers with malaise. Skin testing revealed a positive reaction to hormodendrum. The woman received allergy medications, nasal decongestants and courses of antibacterials. Her symptoms did not resolve. Subsequently, she underwent functional endoscopic sinus surgery, septoplasty and bilateral partial inferior turbinectomy. Initially, her symptoms improved, but her headaches and nasal congestion soon returned. Wellhealed postoperative paranasal sinus cavities were revealed by endoscopic examinations. It was presumed that migraines and allergic rhinitis were contributing factors, and she received medical therapy from allergy, neurology and pulmonary consultants, but her symptoms persisted. In total, she was treated unsuccessfully for symptoms of chronic rhinosinusitis for 12 years. However, upon discontinuation of zolpidem, her symptoms resolved completely. After 9 months, she was still symptom-free and had not been receiving any headache or rhinosinusitis medications. Malloy KM, et al. » Editorial comment: A search of AdisBase and Medline did not reveal any previous case reports of rhinosinusitis associated with zolpidem. The WHO Adverse Drug Reactions database contained four reports of rhinitis and no reports of sinusitis associated with zolpidem.
