Miliary tuberculosis: case report A 42-year-old woman developed miliary tuberculosis while receiving adalimumab [route and dosage not stated] to treat severe psoriatic arthritis. She had been receiving adalimumab for 3 years when she presented with a 3-week history of fevers, weight loss, nonproductive cough, and night sweats. CT scan of her chest showed findings consistent with miliary tuberculosis. Bronchoscopy confirmed Mycobacterium tuberculosis. She received pyrazinamide, ethambutol, rifampicin, and isoniazid. Her symptoms resolved over the next 2 weeks. Author comment: "We describe a patient who developed miliary TB as a complication of TNF-α antagonist therapy despite negative pretherapy screening for [latent tuberculosis infection]."
