Muscle pain: case report A 44-year-old woman developed severe muscle pain during treatment with adalimumab for Crohn’s disease. The woman received an initial dose of adalimumab 160mg, followed by 80mg at week 2; she self-injected adalimumab, provided in pre-filled syringes. She reported severe, generalised pain in her upper and lower extremities 48 hours after receiving the first dose of adalimumab. On examination, she had severe, disabling muscle pain, particularly in her upper arms which were swollen and tender on pressure. No identifiable medical cause for her muscle pain was determined, and she was diagnosed with drug-related muscle pain. Adalimumab was suspected and was therefore discontinued. The woman received ibuprofen and tramadol. Her symptoms subsided over the following week. Fifteen days after the cessation of adalimumab, her muscle pain had resolved completely. Author comment: "Severe [muscle pain] in our patient was deemed to have a probable association with adalimumab." Hinojosa J, et al.
