Alfacalcidol/calcium carbonate Metastatic pulmonary calcification: case report A 66-year-old man developed metastatic pulmonary calcification during treatment with alfacalcidol and calcium carbonate [therapeutic indications not stated]. The man, who was on haemodialysis from past 35 years, presented due to severe multi-joint pain and contractures. He had a history of bilateral carpal tunnel syndrome. He had been receiving alfacalcidol 0.25 µg/day and calcium carbonate 3 g/day [routes not stated]. Laboratory investigations revealed hypercalcaemia, hyperphosphataemia and hyperβ2-microglobulinaemia. A CT scan demonstrated diffuse bilateral lung centrilobular ground-glass opacity and a mass near the coxae and glenohumeral joints. Based on the clinical picture, metastatic pulmonary calcification [durations of treatments to reaction onset and outcome not stated] and dialysis-related amyloidosis were diagnosed.
