Decreased consciousness and haemodynamic instability treated with soya oil emulsion: case report A "middle-aged" woman [age not stated] developed severe baclofen toxicity, with decreased consciousness and haemodynamic instability, after an overdose of baclofen. The woman ingested an unknown quantity of baclofen tablets and was taken to hospital. She was transported by paramedics to a hospital; her Glasgow Coma Scale (GCS) score deteriorated en route [time to reaction onset not stated]. Upon her arrival at the emergency department, her GSC was 8, HR was 65, BP was 101/60 and her RR was 16. Her GCS and BP continued to deteriorate over the next hour. The woman received a fluid bolus, but her condition did not improve. She was intubated, and a 20% solution of soya oil emulsion was administered as a bolus, at a dosage of 1.5 cc/kg. Her BP rose to 89/57 within 30 minutes; she subsequently received an infusion of soya oil emulsion 0.25 cc/kg/min for 30 minutes, and her BP continued to normalise. Her haemodynamic status stabilised within 2 hours of receiving the soya oil emulsion, and she was extubated and discharged the following day.
