Baclofen Various toxicities secondary to drug overdose and abuse: case report

A 16-year-old boy developed coma, confusion, agitation, headache, vomiting, abdominal pain, inconsistent speech, motor restlessness and drug toxicity secondary to baclofen overdose while taking it for recreational use i.e. drug abuse [amount consumed for abuse not stated]. The previously healthy boy was admitted to the emergency department with confusion and marked agitation. He had a 48 hours history of headaches, vomiting and a diffuse abdominal pain, followed by the development of agitation and confusion. Prior to the presentation, several empty blisters of baclofen were found in his room. His medical record revealed a coma episode that developed several months previously. At that time, the aetiology of coma was unknown. Upon admission, he had an acute confusional state characterised by inconsistent speech and motor restlessness. His Glasgow coma scale score was 10/15. His HR was 40 bpm and BP was 159/85mm Hg. The rest of the clinical examination was unremarkable. All other laboratory investigations were within normal ranges. Thereafter, plasma and urine samples were collected for toxicological analysis. The initial screening using liquid chromatography with diode array detection (LC-DAD) and urine analyses by gas chromatography with mass spectrometry detection (GC-MS) of plasma and urine yielded negative results. Baclofen was detected in plasma at a concentration 420 ng/mL and 64 900 ng/mL in urine, indicative of baclofen overdose and [time to reaction onset not clearly stated]. Following symptomatic management, the boy’s condition remarkably improved, and he was eventually discharged to an adolescent psychiatric unit. Psychiatric assessment revealed that he had recreationally used baclofen. Addionally, coma episode in the past was also attributed to one of the baclofen abuse session. Drevin G, et al.
