Lack of efficacy: case report A 54-year-old woman exhibited lack of efficacy while receiving palliative therapy with bevacizumab and doxorubicin liposomal for recurrent clear cell ovarian cancer and inflammatory breast cancer. The woman presented with complaints of vague anterior chest and breast pain in October 2019. An anamnesis showed that she was diagnosed with a left ovarian neoplasm (high-grade FIGO stage IIIA) in 2018 and had undergone laparotomy with total abdominal hysterectomy, bilateral salpingo-oophorectomy and omentectomy. At that time, she had received 6 cycles of paclitaxel and carboplatin between 15 October 2018 and 28 February 2019 with improvement. The dose of last three cycles were decreased due to neuropathy and neutropenia [aetiology not stated]. At the time of current presentation, various investigations led to a diagnosis of inflammatory breast cancer. Biopsy was consistent with recurrent metastatic carcinoma of ovarian origin. Her inflammatory breast cancer was attributed to recurrent clear cell ovarian cancer. She started receiving palliative chemotherapy with doxorubicin liposomal and bevacizumab on 5 January 2020, followed by radiation therapy to the breast. Post-radiation, she developed pneumonia with para-pneumonic effusion due to the radiation therapy. Because of progressive back pain, MRI pelvis was performed, which revealed multiple variable-sized focal osseous lumbar vertebral and bony pelvis likely metastatic. She therefore received one session of radiotherapy to for pain control. Despite chemotherapy, her general condition continued to worsen along with poor performance status, which indicated lack of efficacy to bevacizumab and doxorubicin liposomal. The woman’s palliative chemotherapy was stopped due to the worsening of her general condition. Initiation of supportive measures was planned; however, she wanted to go back to her home country to continue her treatment there. Abu-Tineh M, et al.
