Hypocalcaemia and QT interval prolongation secondary to overdose: case report A 4-year-old girl developed marked hypocalcaemia and prolongation of QT interval secondary to accidental overdose of calcium phosphate. The girl was admitted following an accidental ingestion of 200 tablets of calcium phosphate [calcarea phosphorica] [dosage not stated]. Although she was asymptomatic, further investigations showed marked hypocalcaemia (undetectable ionised calcium); however, no other electrolyte abnormalities were noted. ECG findings were significant for QT prolongation (550ms) [time to reaction onsets not stated]. The girl underwent gastric lavage and received calcium correction twice. Consequently, the level of ionised calcium normalised, and subsequent ECG showed normal QT interval (420ms). Thereafter, she was initiated on calcium maintenance. Further investigations showed normal levels of ionised calcium. Complete blood count, liver and renal function tests were unremarkable. Eventually, she was prescribed oral calcium supplementation and discharged on day 2. She remained asymptomatic thereafter. Bhattacharya D.
