Clarithromycin Hypomania in a child: case report A 3-year-old boy developed hypomania during treatment with clarithromycin for community-acquired bacterial pneumonia. The boy presented with low-grade fever, cough, loss of appetite and general malaise and was diagnosed with pneumonia. He started receiving oral clarithromycin 7.5 mg/kg twice daily as empirical therapy for communityacquired bacterial pneumonia. After his second dose, he developed irritability and hyperactivity. Several hours after his third dose, his irritability had increased and he had pressured speech, aggressive behaviour, psychomotor agitation and insomnia. He got bored quickly and was continually active. Auscultation revealed persistence of the crepitant rales. He was diagnosed with an episode of hypomania. Clarithromycin was considered the only possible cause of the hypomania and was discontinued. Over 36 hours, his symptoms gradually abated. He then received amoxicillin/ clavulanic acid without any recurrence of hypomania. Author comment: "The rapid onset of the hypomanic episode after clarithromycin intake, its resolution once the drug was stopped, and the absence of other possible causative factors favoured the diagnosis of a clarithromycininduced hypomanic episode.
